Jeremiah Shay of 512 Third Street, Towanda, PA died 2 July 1937 at 11:35 a.m. He was in the Robert Packer Hospital in Sayre, Bradford County, Pennsylvania. Cause of death was listed as "acute gangrenous appendicitis, perforated", which began on 18 June 1937.

The appendectomy was performed on 21 June and he contracted hypostatic pneumonia on 29 June.

His age is noted to be 79 years, 11 months, and 19 days. His wife is named as Jane Grady Shay. He was born in Ireland on 13 July 1857.

His parents, both born in Ireland, were Patrick Shay [sic] and "Ellen ???".

The informance is listed as a Robert Packer Hospital employee, but the information almost certainly came from one or more of his children.
